Center Pointe Family Medicine Patient Info
In order to make your transition to Center Pointe Family Medicine as smooth and stress free as possible, we ask that you preregister via our patient portal prior to your first appointment. If you would prefer to register using our paper format instead of electronic format, please print and fill out the New Patient Registration Forms and bring them with you to your first appointment. We also ask that you bring the following items to your first appointment:
- Current copy of your insurance card.
- Photo ID (if patient is a minor then parent/legal guardian ID)
- Co-pay that is due at the time of service
- List of current medications that you are taking and medical records from previous provider including vaccination/immunization record.
- Copies of any Patient Registration Forms that you filled out.
